Sometimes a family member communicates with their loved one living in a senior community, and realizes something is wrong. Many times patients are affected by chronic conditions such as  stroke, paralysis, or the development of advanced dementia or parkinsons disease. This becomes a difficult transportation & logistical problem for family members, shuttling their loved one to & from multiple physician offices, hospitals, and therapy facilities. This is especially true for out of town family.

At our practice we make all services, including the physician on-site. That way you don’t have to miss work and there are no weather issues or need to rely on transportation services. Whether you need consistent home evaluations or occasional check-ins, we provide this service as the ultimate convenience for your already hectic life.  We often are at a senior community on a weekly basis, if not more, allowing us to check on your family member with ease and frequency, and report back to you.
